Depending on Office Macro security settings and/or Protected View settings you may need to enable macros or place the file in a Trusted location to enable the AddIn to work. In most cases adding it in the AddIns Manager will also resolve those issues. See the bottom 2 paragraphs for how to do load the AddIn with the AddIns Manager in Excel.

What is Protected View?

When XSFormatCleaner.xla is opened in Excel 2003 there will be a new item on the File menu which will say
"Clear Excess Formats in (bookname)"

When XSFormatCleaner.xla is opened in Excel 2007, 2010 or 2013 there will be a new item on the Add-Ins tab which will say
"Clear Excess Formats in (bookname)"

If you use the functionality often you can use Excel's AddIn Manager to load the AddIn with Excel.
In Excel 2003 use Tools | Add-Ins... to launch the AddIns manager.
In Excel 2007, 2010 or 2013 use altti to launch the AddIns manager (or in Excel Options use Manage Excel Add-Ins on the AddIns dialog).

Once the Add-Ins dialog is open use the Browse... button to browse to the location of XSFormatCleaner.xla, then click OK twice.

Works fine for me in Excel 2010 - can't get it to work in Excel 2013 on multiple machines - it appears to be installed in File->Options->Addins but it doesn't appear on the Ribbon at all. Anyone have any ideas ?

Ah silly me! I placed the file in an untrusted location. Once I fixed that, it worked.

In Excel 2010 under Windows 7 Pro, attempting to enable the Add-In results in:
"Office has detected a problem with this file. To help protect your computer this file cannot
be opened."
followed by a 'Show Help >>' button which displays:
"The Office File Validation feature has blocked this file from opening as it may contain harmful
content."
This occurs even when the downloaded file is unblocked prior to copying it into a Library...
